Society at large has a dysfunctional relationship with the mental illness colloquialism 'nuts.' Basically, whether or not it is being used endearingly or pejoratively is entirely situational, and usually dependent upon the social acceptability of the subject that one is 'nuts' over.
This shirt has a cuddly, cartoon squirrel on it though, so it's probably in the acceptable category.
